Abstract: Brain stem involvement was proposed early on during the SARS-CoV-2 pandemic as a possible concurring cause of respiratory failure, given that the virus showed the capability of inducing neurological damage, and additional factors beyond direct pulmonary damage appeared to play a role in the clinical picture. In addition, autoimmune neurological complications of SARS-CoV-2, both acute and chronic, are well documented and COVID-19 appears to be associated with the hyperactivation of the immune system and a cytokine storm, thus resembling other clinical conditions characterized by immune dysregulation and autoantibody production. Recent evidence points to autoimmune antibody-mediated brain stem damage in COVID-19 both in the acute phase of the disease as well as in the long-term chronic sequelae.
