Abstract: Early perfusion amyloid-PET imaging is emerging as a valuable tool to potentially capture patients’ neurodegeneration patterns, allowing same-day, same-tracer categorization of both amyloidosis (A) and neurodegeneration (N) biomarkers. Early perfusion amyloid-PET might provide an added value for differential diagnosis and, in keeping with the capability of FDG-PET to indirectly track co-pathology in AD patients, it may complement the information provided by amyloid PET thus supporting a more accurate weighting of the different underlying pathological processes. Early perfusion imaging is, at the moment, still a research tool, as high-level evidence regarding a non-inferiority with respect to FDG-PET (especially in MCI patients) is still not available due to the lack of dedicated clinical trials. The present paper states the position of EANM on early perfusion imaging in the framework of dual-point amyloid-PET studies. The mounting evidence for its use, as well as unmet needs and potential cost-effectiveness, are summarized and discussed with the aim of paving the path for a proper recognition of its added value in the AT(N) system and in the clinical setting to further support differential diagnosis.
