Abstract: Background: White matter (WM) alterations are a commonfeature of Alzheimer Disease (AD), typically detected in vivowith diffusion tensor imaging (DTI). Blood-based neurofila-ment light (NfL) is an emerging biomarker for monitoringneurodegeneration. While NfL is thought to reflect damageto myelinated axons, minimal work has directly examinedthis association. The objective of this study was to examinethe cross-sectional and longitudinal relationship betweenWM neuroimaging markers and serum NfL levels.
